Independence, Financial Transparency and Patient Choice

Healthspan Collective is a collaborative model designed to support coordinated, multidisciplinary care between independent healthcare providers, including MAN Health Clinic, Adapt Health Club, and Catalyst Dietitian.  Each provider operates as a separate and independent entity. There is no partnership, employment relationship, joint venture, fee-sharing arrangement, commission, referral payment, or financial incentive between participating providers. Each practitioner is solely responsible for the services they provide and for the clinical advice, treatment, or recommendations they deliver.

Where appropriate, providers may recommend that patients consider services offered by other practitioners within the Healthspan Collective. These recommendations are made on the basis of clinical judgement and patient benefit only. Patients are under no obligation whatsoever to proceed with any recommended provider, investigation, or service, and are free to decline, defer, or seek alternative providers at any time.

All services are billed separately by each provider. Payment for services is made directly to the provider delivering that service under their own fees policy arrangements.

Participation in Healthspan Collective does not replace the importance of maintaining an ongoing relationship with your regular care team or other healthcare providers. By choosing to engage with any provider within Healthspan Collective, you acknowledge and understand the independent nature of each practitioner and the collaborative, non-commercial structure of the model.

Specific information about each provider’s fees, policies and scope of practice must be sought from the provider themselves.
